Siouxland Community Health Center has an opening for:
Community Health Worker
1 - Full-time

Schedule would be: Monday 8am-5pm, Tuesday OFF, Wednesday 8am-8pm, Thursday 8am-5pm, & Friday 8am-5pm

Successful candidate must have solid experience & advanced knowledge of:
  • High School diploma or GED.
  • Advanced computer and data entry skills
  • Valid driver's license
  • Basic Life Support Certification (can be obtained after hire at SCHC)
  • Mandatory Reporter Certification for Child & Dependent Adult (can be obtained after hire at SCHC)
  • Complete Professional Skills Training for Community Health Workers within a year of hire.
  • Complete health coach training (Fusion) and pass motivation proficiency within two years of hire.
Successful candidate must be able to perform primary functions of position:
  • Assist patients in navigating health system at the Health Center, County, or State level.
  • Access and communicate barriers in navigation to superiors.
  • Provide basic education on need for screening tests, self- management of chronic diseases/wellness.
  • Contact patients under supervision of clinicians to assess the need for additional services, compliance with treatment programs/plans.
  • Travel to patients' homes to assess barriers and help coordinate the reduction of those barriers.
  • Assist quality staff with data projects and assist marketing staff with community events.
  • These functions are not all inclusive
